LICENSING REQUIREMENTS

DEALER TRADE NAME CHANGE INSTRUCTIONS

1. COMPLETE, SIGN AND HAVE NOTARIZED THE ENCLOSED VEHICLE DEALER APPLICATION INDICATING THE NEW TRADE NAME. APPLICATIONS MUST BE COMPLETED AND RETURNED TO THIS OFFICE BY THE DEALER. APPLICATIONS WILL NOT BE ACCEPTED FROM INSURANCE COMPANIES OR TITLE COMPANIES.

2. MUST HAVE A BUSINESS TELEPHONE LISTED IN THE BUSINESS NAME AND INSTALLED AT THE PLACE OF BUSINESS WHICH IS LISTED ON THE APPLICATION. NO CELLULAR PHONES.

3. OBTAIN A CHANGE RIDER FROM YOUR BONDING COMPANY INDICATING THE BOND BE AMENDED TO READ - PRINCIPAL(S) DBA NEW TRADE NAME.

4. OBTAIN AN ENDORSEMENT FROM YOUR INSURANCE COMPANY INDICATING THE LIABILITY INSURANCE POLICY BE AMENDED TO READ - NEW TRADE NAME.

5. SUBMIT A PHOTOGRAPH OF YOUR BUSINESS SIGN STATING THE NEW TRADE NAME. THE SIGN MUST BE A MINIMUM OF 16 SQUARE FEET(4' X 4') AND SUBJECT TO ALL ZONING LAWS.

6. SUBMIT $100.00 TO COVER THE TRADE NAME CHANGE FEE.

7. SUBMIT ALL YOUR CURRENT LICENSES AS NEW LICENSES WILL BE PRINTED LISTING THE NEW TRADE NAME.
