APPLICATIONS WILL NOT BE ACCEPTED FROM INSURANCE COMPANIES
OR TITLE COMPANIES.

HAVE A BUSINESS TELEPHONE LISTED IN THE BUSINESS NAME AND
INSTALLED AT THE PLACE OF BUSINESS WHICH IS LISTED ON THE
APPLICATION. NO
CELLULAR PHONES.

COMPLETE, SIGN AND HAVE NOTARIZED THE ENCLOSED SALESMAN
APPLICATION FOR THE ADDITIONAL LOCATION. YOU MUST HAVE AT
LEAST (1) LICENSED SALESMAN AT EACH LOCATION.
4. OBTAIN A
CHANGE RIDER FROM THE BONDING COMPANY ADDING THE PHYSICAL
ADDRESS OF THE ADDITIONAL LOCATION. THE RIDER MUST ALSO
INCLUDE THE TRADE NAME CHANGE IF IT IS DIFFERENT FROM THE 1ST
LOCATION.
5. OBTAIN A
CERTIFICATE OF INSURANCE INDICATING THE PHYSICAL ADDRESS AND
THE NAMES OF ALL SALESMEN LICENSED AT THE ADDITIONAL
LOCATION.
6. COMPLETE
THE ENCLOSED VERIFICATION OF PROPER ZONING FORM INDICATING
THAT YOU CAN OPERATE THIS TYPE OF BUSINESS AT THE ADDITIONAL
LOCATION.
7. SUBMIT A
PHOTOGRAPH OF YOUR BUSINESS SIGN STATING YOUR TRADE NAME AT
THE ADDITIONAL LOCATION. THE SIGN MUST BE A MINIMUM OF 16
SQUARE FEET (4' X 4') AND SUBJECT TO ALL ZONING LAWS.
8. SUBMIT
$100.00 TO COVER THE ADDITIONAL LOCATION FEES AND $25.00 FOR
EACH SALESMAN LICENSE.
9. A
PHYSICAL INSPECTION WILL BE MADE BY THIS OFFICE.
